Donor Appreciation Giveaway 2008

By participating in the Virginia Blood Services/MIDAS of Richmond Giveaway, participants agree to accept and be bound by these rules and the decisions of MIDAS of Richmond and Virginia Blood Services as which are final in all matters relating to the Giveaway.

1. Participants are eligible to receive Giveaway awards by registering to donate blood at a VBS Mobile Blood Drive held at a designated MIDAS location during the MIDAS of Richmond, Inc. Donor Appreciation Giveaway 2008 eligibility period.

2. Participants must be over the age of 18 and eligible to donate blood or blood products and must register to donate at a VBS Mobile Blood Drive Unit at a designated MIDAS location during one or more of the following Giveaway drive events.

Eligibility Period: January 12, 2008 to December 27, 2008

Anticipated Drive Dates:

• January 12, 2008
• March 8, 2008
• April 5, 2008
• June 28, 2008
• August 30, 2008
• November 22, 2008 and
• December 27, 2008

Anticipated Drive Locations: Drives will be held at the following MIDAS stores:

• 11463 W. Broad St, Richmond
• 1400 Boulevard, Colonial Heights
• 10160 Hull St., Richmond, and
• 5527 Mechanicsville Turnpike, Mechanicsville.

Virginia Blood Services and MIDAS reserve the right to change or eliminate any Drive date or location.

3. Each participant will receive one Giveaway award for the Giveaway drive in which they have registered and will receive one premium Giveaway award if they have previously participated at one or more earlier qualifying Giveaway drive events at a MIDAS location during the Donor Appreciation Giveaway eligibility period.

4. Participants are encouraged to make appointments for donations. Walk-ins will be accommodated only at the discretion of Virginia Blood Services. Prospective donors who cannot be accommodated because of staff limitations or for other reasons will not be eligible to receive Giveaway rewards.

5. Virginia Blood Services and/or MIDAS of Richmond are not responsible for lost, stolen, or misplaced Giveaway cards.

6. All participants agree to release, hold harmless and indemnify Virginia Blood Services and MIDAS of Richmond, Inc. from and against any and all losses, damages, claims, suits and causes of action arising from and/or related to this Giveaway and/or any of the awards awarded pursuant thereto.

7. Giveaway Award Descriptions:

1st Donation: Oil Change.

2nd and 3rd Donation: Oil Change and choice of a premium level 1 award (State Inspection, Wiper Blades, Tire Rotation or Air Filter.

4th Donation: Oil Change, choice of a premium level 1 award (State Inspection, Wiper Blades, Tire Rotation or Air Filter) and participants who have registered to donate at four Giveaway drives may submit their completed Giveaway card as provided herein and will be eligible to receive a set of two tires. Fifteen sets of two tires will be awarded to fifteen participants, each of whose Giveaway card is drawn at random from all eligible participants.

5th Donation: Oil Change, choice of a premium level 1 award (State Inspection, Wiper Blades, Tire Rotation or Air Filter) and participants who have registered to donate at five Giveaway drives may submit their completed Giveaway card as provided herein and will be eligible to receive a set of four tires. Fifteen sets of four tires will be awarded to fifteen participants, each of whose Giveaway card is drawn at random from the eligible participants.

Appointments are required for giveaway redemption.

8. Giveaways awards may be claimed only by, and will be awarded only to, the individual identified on the Giveaway card. Appointments are required for giveaway redemption. The Giveaway card holds no cash value and cannot be substituted or exchanged for any other prize or award or redeemed for cash. Donations at other VBS locations and mobile drives are not considered for this Giveaway.

9. In order for a participant to participant in a Giveaway drawing for a set of two tires or a set of four tires, the participant must submit his or her Giveaway card, showing participation in four or five donations, to a MIDAS of Richmond, Inc. facility on or before December 31, 2008. Drawing winners will be notified by MIDAS of Richmond, Inc. by telephone. If MIDAS of Richmond, Inc. is unable to locate or contact a winner or the winner does not reply to the notification within fourteen days, the Giveaway, at the sole discretion of MIDAS of Richmond, Inc., will awarded to an alternate winner. Giveaway drawings will be conducted on or before January 31, 2009.

10. All federal, state and local taxes are the sole responsibility of the winner.

11. Virginia Blood Services and MIDAS of Richmond, Inc. reserve the right to disqualify any individual they determine and/or deem to be tampering or improperly affecting the entry process or the operation of the Giveaway.

12. Virginia Blood Services and MIDAS of Richmond, Inc. reserve the right to cancel, terminate, or suspend the Giveaway as they may deem appropriate.

13. All entrants agree and acknowledge that in the event of any dispute or disagreement arising out of participation in this Giveaway, including, but not limited to, disqualification of entrants, awarding of prizes, value and quality of prizes, or any damage, injury, or claim associated with the use of prizes, shall be resolved individually and not by a class action lawsuit or any action under Va. Code § 8.01-267.1 or any similar provision thereto.

14. Non-compliance with any rule, requirement or mandatory procedure herein will, at the sole discretion of Virginia Blood Services and/or MIDAS of Richmond, Inc., result in disqualification.

15. The Giveaway is subject to any requirements, revisions and/or limitations imposed by applicable federal, state or local law.

16. The odds of winning any prize are dependent on the number of eligible entries received.

17. All Giveaway awards must be claimed by March 31, 2009.

18. Participants acknowledge that redemption must be made in accordance with the rules, limitations and restrictions imposed by MIDAS of Richmond, Inc. Virginia Blood Services and MIDAS of Richmond, Inc reserve the right to change the Giveaway awards at any time.
